Main Duties:

  • Work within a strengths-based model of practice, managing a non-complex caseload to support residents in improving their independence and wellbeing by collaborating closely with partners and maximising the use and benefits of community resources.
  • Conduct strengths-based assessments, reviews, and support planning to enable residents to maximise their independence, enhance their quality of life, and build resilience.
  • Provide practical support to residents and carers, including helping them access living aids and the welfare benefits system, and offering advice and information to service users.
  • Assist in arranging short-term care and provide clients and carers with respite services to prevent carer breakdown.
  • Work with adult clients, relatives, carers, and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to support planning.
  • Liaise with colleagues across the Haringey Health and Social Care Partnership to gather information relevant to assessment and support planning activities.
  • Maintain and update reliable, timely, and accurate case notes, systems, and other records, and write reports as required; support social workers in collecting information and presenting it for legal purposes.
  • Undertake reviews of care packages.
  • Contribute to effective multi-disciplinary team working by participating in meetings and collaborating with others to achieve the best outcomes for service users.
  • Attend safeguarding strategy meetings when required.
  • Complete Continuing Health Care checklists and identify those cases that require full assessments.

Requirements of the Social Work Officer:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
