- Provide valuable medication support, monitoring and ensure continuity of care at home and in the community.
- Focus on maintaining and observing patients' mental health status, tailoring interventions to individual needs.
- Collaborate with a multidimensional team, sharing expertise and insights to enhance overall service provision.
