What you will do:
- Monitor daily health observations and swiftly identify any concerns or changes, playing a crucial role in preventing hospital admissions.
- Attend weekly multidisciplinary meetings, actively participating to provide feedback and support the progression of clients’ care.
- Implement care and rehabilitation plans drawn up by professionals such as Physiotherapists and Occupational Therapists, ensuring that every client receives personalized and effective care.
- Communicate regularly with team leads to offer insightful feedback, aiding in the review and adjustment of care and rehab plans to better meet clients' needs.
Requirements of the Therapy Assistant
- Recent experience working as a Physiotherapy/Occupational Therapy/Therapy Assistant
- NHS experience would be advantageous but not essential
