Registered Mental Nurse - Band 6 - Bed Coordinator

What you will do:
- Collaborate closely with the Out of Area bed manager to manage SHSC patient caseloads.
- Liaise with external providers to evaluate the care and progress of patients in out of area placements.
- Attend and actively participate in patient MDT meetings and complex case meetings, gathering essential clinical updates.
- Challenge care and discharge decisions effectively using support and escalation processes when necessary.
- Work with inpatient and community teams to facilitate timely patient repatriation or discharge.
- Enable unit transfers to best meet patient needs.
- Make appropriate service referrals on behalf of patients.
- Arrange and oversee 48-hour and 7-day follow-ups with the necessary teams.
- Ensure all safeguarding concerns are promptly escalated.
- Offer expert advice to inpatient and community teams.
- Engage with the SHSC flow team to discuss patient prioritization and facilitate smooth patient flow.
- Update SHSC leadership on Out of Area bed status in meetings.
- Provide contracting and BPM teams with accurate information when needed.
- Offer supervision to junior staff when required.
