Main Duties:

  • Provide professional leadership and clinical supervision to the Home 1st Therapists, ensuring high standards of care and effective team performance.
  • Coordinate interdisciplinary assessments to prevent avoidable hospital admissions and readmissions across various settings.
  • Communicate effectively and collaborate with other Home 1st leads to ensure comprehensive patient care.
  • Conduct advanced therapy assessments and deliver treatment to service users.
  • Contribute to the ongoing development and improvement of the service.
  • Offer expert clinical advice and support for complex hospital discharges.
  • Hold a caseload of patients, providing individualised care and treatment plans.
  • Provide professional and personal support to Home 1st staff, fostering a cohesive team environment.
  • Implement specialised therapy treatments and programmes, using analytical and judgement skills to address therapy-related issues.
  • Obtain valid informed consent and work within legal frameworks for individuals lacking capacity to consent.
  • Encourage family and carer involvement in decision-making and planning of care objectives.
  • Collaborate with other Home 1st leads to ensure coordinated multidisciplinary activities and adapt work practices as needed.
  • Develop and amend team policies that impact the multidisciplinary team.
  • Support the therapy team in daily management tasks, including recruitment, appraisals, and performance management.
  • Ensure the safe and competent use of all equipment, initiate regular audits, and oversee training and competency evaluations for unqualified staff.
  • Maintain and update client records using an electronic database.
  • Monitor, prioritise, and allocate work across the therapy team.
  • Establish and maintain partnerships with Milton Keynes University Hospital Foundation Trust (MKUHFT) and Milton Keynes Council (MKC) to improve services and achieve organisational objectives.
  • Lead on CQC activities in partnership with the team, ensuring compliance with NICE guidance.
  • Ensure effective systems are in place for assessment, care planning, and review of intermediate care service referrals, supporting interdisciplinary work and person-centred care planning.
  • Understand and implement clinical and HR policies and procedures within CNWL.
  • Ensure therapists have personal development plans and receive annual appraisals.
  • Coordinate duty rotas, annual leave, and training schedules to maintain adequate staffing levels.
  • Adhere to and enforce all agreed policies and procedures, including health and safety regulations.

Requirements of the Physiotherapist:

  • Qualified physiotherapist, i.e., Degree or equivalent within Physiotherapy
  • Recent UK experience within community physiotherapy
  • Registered with the HCPC as a physiotherapist to work in the UK
  • NHS experience would be advantageous but not essential
