What you will do:

  • Provide assessment and treatment for patients within their homes or suitable community settings, focusing on neuro and stroke rehabilitation.
  • Develop and implement personalised treatment plans to aid recovery and maximise independence.
  • Work collaboratively with multidisciplinary teams to ensure holistic patient care.
  • Keep clear and concise records of patient progress, noting improvements and areas needing further intervention.
  • Participate in team meetings and contribute to case discussions for holistic care planning.


Requirements:

  • You need to have a minimum of one year's UK or NHS experience in a similar role.
  • Being a car driver is essential due to the community-based nature of the role.
  • An HCPC registration is required, ensuring you meet professional standards and are recognised to practise in the UK.
