What you will do:
- Lead and provide clinical supervision to the Home 1st Therapy team.
- Coordinate interdisciplinary assessments to prevent avoidable hospital admissions and readmissions.
- Collaborate effectively with MKUHFT, MKC, and third-sector participants to streamline patient pathways.
- Communicate clearly with other Home 1st leads to ensure patients' needs are addressed efficiently.
- Deliver advanced therapy assessments and treatments for service users.
- Contribute to ongoing service development to maintain excellence in care delivery.
- Partner with professional leads for therapies in MK to promote the development of therapy staff.

Requirements:
- HCPC Registration
- UK experience working as an Occupational Therapist
- Degree/Equivalent in Occupational Therapy
- Car driver with valid licence to facilitate community visits
