Location: Havering. Hybrid at least 3 days onsite. Flexible approach involving working from our town hall campus/hubs. Expectation to work from our town hall campus during your team's duty week, for team meetings and other development days and at the manager's discretion some ad-hoc days onsite may be needed. Encouraging face-to-face working for teams as we know how important this is for workers and all meetings with children and family take place face to face.

Duties:

  • Manage and develop positive working relationships with young people living locally and nationally, empowering young people to participate in all decision-making processes affecting their lives and ensure feedback to shaping developments in the leaving care service.
  • Hold caseload responsibility for children and young people ensuring comprehensive assessments of needs, strengths, and risks are undertaken in timescales.
  • Develop, monitor and review holistic pathway plans in timescales to ensure young people’s identified needs are met and supported by setting realistic goals and using appropriate interventions to help care leavers achieve their potential.

Requirements of the Leaving Care Pathway Co-Ordinator:

  • A relevant practice qualification in childcare, education, health or social care, e.g. NVQ 3 or equivalent/higher or additional qualifications/training in specialist areas including child protection. 
  • Significant experience in Social Services within the UK
