Sanctuary Personnel is currently looking for an experienced Band 6 Community Psychiatric Nurse (CPN) to take on a new ongoing contract role within the London Team.

Location: South West London

Pay Rate: £30 per hour (assignment rate)

Contract Type: Locum, 6-month contract/ongoing block booking.

Working hours: Monday – Friday, 37.5 hours per week (hours flexible between 8am and 8pm). Part time will be considered.

Main Duties:

  • Caseload Management:

    • Manage a designated caseload flexibly to meet the unique needs of each client and their carers.
    • Ensure high standards of personalised care, recognising factors that impact clients' mental, emotional, and physical well-being, and provide appropriate support and advice.
  • Care Coordination:

    • Act as a care coordinator by assessing clients’ needs and strengths collaboratively.
    • Ensure all clients have care plans that are implemented and regularly reviewed or updated.
  • Practice Standards:

    • Develop and maintain high standards of practice using a range of psychosocial approaches within a health and psycho-social framework.
  • Communication and Liaison:

    • Communicate and liaise effectively with relevant agencies to ensure an integrated care programme.
  • Medication Management:

    • Manage medication in accordance with Trust policy and NMC standards for drug administration.
  • Client Processes:

    • Organise and participate in the admission, discharge, and transfer of clients, following Trust policies.
  • Information Provision:

    • Provide clients and carers with up-to-date verbal and written information about Trust services, how to express their views, the complaints procedure, and what to expect from the CMHT.
  • Clinical Meetings:

    • Attend weekly clinical meetings to share relevant information about caseloads and provide updates on new assessments.
  • Medication Monitoring:

    • Supervise and monitor the therapeutic effects and side effects of prescribed medication, offering clear advice and information to clients.
  • In-Patient Coordination:

    • Develop and maintain effective working relationships with in-patient facilities for the admission and discharge of clients.
  • Carer Support:

    • Facilitate carer assessments, develop appropriate care plans to meet identified needs, and monitor their effectiveness.

Requirements of the Community Psychiatric Nurse:

  • Degree or equivalent in Mental Health Nursing
  • Experience working as a Mental Health Nurse
  • NMC Registration
