What you will do:

  • Conduct comprehensive assessments of service users' needs, developing person-centred care plans and providing evidence-based interventions.
  • Manage an active caseload under the Care Programme Approach, collaborating closely with multidisciplinary teams to deliver cohesive and holistic care.
  • Identify potential risks, implement effective safeguarding measures, and offer robust support to service users during crises to prevent hospital admissions.
  • Actively work alongside psychiatrists, social workers, GPs, and external agencies to ensure a seamless delivery of care.
  • Encourage active engagement with service users in their care planning process while offering guidance and support to families and carers.
  • Maintain precise and up-to-date records, ensuring compliance with legal frameworks such as the Mental Health Act and safeguarding policies.
