What you will do:
- You will assess needs, and plan, and deliver comprehensive care from assessment through to discharge, all while maintaining a person-centred approach.
- Craft and evaluate individualised care plans, in collaboration with service users, carers, and other professionals, to ensure tailored support.
- Work closely to empower mental health service users in community settings, supporting them on their recovery journey.
- Act as an advocate for individuals using the service, ensuring their needs and perspectives are respected and championed.
- Provide supervision and guidance to non-qualified staff, enhancing team development.
- Collaborate effectively with external partners, including social services and healthcare agencies, to offer holistic care and interventions.
- Engage in therapeutic interventions and signpost additional resources as needed.
- Maintain a basic understanding and application of the Mental Health Act to inform and support clinical practice.
