What you will do:
- Provide essential nursing care to patients, assessing their needs and developing, implementing, and evaluating care plans.
- Support the District Nursing Team with scheduled and unscheduled care for patients with complex needs.
- Liaise with GPs and other healthcare professionals to review and support patient care plans.
- Communicate effectively with patients, families, and healthcare colleagues, ensuring a high level of patient confidentiality and care.
- Participate in health promotion activities and encourage patients to manage their own health effectively.
- Ensure accurate documentation of patient care and maintain meticulous records in line with organisational policies.
- Advocate for patients, ensuring they receive the support and care they need.
