Key Responsibilities:

  • Comprehensive Assessments: Conduct assessments of patients’ physical, mental, and environmental needs to develop personalised treatment plans aimed at enhancing independence and improving quality of life.
  • Therapeutic Interventions: Design, implement, and evaluate interventions to address a range of conditions, improving patients' functional abilities in daily living activities.
  • Multidisciplinary Collaboration: Work closely with other healthcare professionals, including physiotherapists, doctors, nurses, and social workers, to provide holistic and coordinated patient care.
  • Patient Education: Educate patients and their families on strategies to manage conditions, improve independence, and utilise adaptive equipment or assistive technology to enhance daily living activities.
  • Caseload Management: Manage a caseload, ensuring accurate documentation of patient progress, treatment plans, and any changes in condition. Regularly review and update care plans according to patients' evolving needs.
