What you will do:
- Conduct triage assessments and manage a caseload while offering short-term interventions.
- Deliver a range of evidence-based psychosocial interventions for service users on a specific caseload.
- Work collaboratively with partner agencies, including Primary Care, Secondary Care, MIND, and other community services.
- Use shared decision-making within the MDT to ensure safe and effective discharge planning or onward referrals.
- Provide psychological support, social prescribing advice, and medication management to service users.
- Support families and carers in managing risk and escalate concerns as required.
- Ensure all care planning is shared promptly with relevant parties and aligns with operational guidelines.
Skills and Requirements:
- Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist with relevant professional registration.
- Previous experience in community mental health settings is essential.
- Strong IT skills in line with NHS Band 6 requirements.
- Ability to work independently while contributing effectively to a multidisciplinary team.
- Excellent communication skills and the ability to build relationships with patients, carers, and partner organisations.
