What you will do:
- Conduct assessments and reviews in line with the Care Act 2014 and the Mental Capacity Act 2005 to ensure person-centered and effective service delivery.
- Lead and complete safeguarding enquiries, ensuring the wellbeing and protection of vulnerable adults.
- Utilise a strengths-based approach to help clients maintain independence and strengthen community connections.
- Collaborate closely with colleagues on a duty rota, sharing responsibilities to provide comprehensive support.
- Document and maintain records using the Mosaic client database, contributing to steady and accurate reporting.
