You will be working full-time with weekdays of 9-hour days, from 08:30 to 17:00, every Monday and Wednesday.

What you will do:

  • Conduct comprehensive face-to-face new patient assessments and community assessments within the comfort of patients' homes.
  • Execute telephone assessments efficiently to provide essential support and guidance.
  • Manage bladder scanning, catheter care, and TWOC procedures.
  • Complete annual pad reviews and initiate discharge reports as needed.
  • Collaborate with GPs and initiate referrals, ensuring a seamless healthcare experience for patients.
  • Administrative duties include the completion of assessments on SystmOne or EMIS and communication with patients and healthcare providers.
  • Staff will have to submit a service delivery report for each day worked.
  • Staff will have to submit subsequent report for any DNAs or Urgent patients


Requirements:

  • Applicants must be a registered nurse with community assessment experience at band 6.
  • Familiarity with bladder scanning, catheter management, and TWOC is essential.
  • Training in SystmOne or EMIS community modules, approximately 3 hours, will be required virtually with possible onsite orientation.
