Main duties/responsibilities:
- Face-to-Face (F2F) Home Visits: Conduct regular home visits for patients with conditions such as Stroke, Acquired Brain Injury (ABI), Parkinson’s, Motor Neurone Disease (MND), and Multiple Sclerosis (MS). Independently manage your diary and schedule bookings for patient visits.
- Clinical Expertise: Provide expert care in managing patients with dysphagia (swallowing disorders), aphasia, and motor speech disorders. Perform detailed speech and language assessments to develop personalized care plans for patients.
- Documentation and Communication: Write and send an initial letter to the patient’s GP following the first assessment to outline the patient's status and care plan. Upon completion of the treatment, prepare and send a discharge letter to the GP summarizing progress and recommendations. Maintain accurate, detailed, and paper-based records for all assessments and patient interactions.
- Multidisciplinary Team (MDT) Meetings: Attend and participate in the weekly MDT meeting every Tuesday from 9:00 AM to 10:00 AM to discuss patient progress and collaborate with the healthcare team.
- Daily Workload Communication: At the beginning of each workday, send an email to the team detailing your daily workload and planned activities. This role requires strong organizational and communication skills, as well as a high level of clinical expertise in managing speech and language disorders associated with neurological conditions.
Requirements of the Speech and Language Therapist/SLT:
- Degree or equivalent within Speech and Language Therapy and HCPC registration
- Recent UK/NHS post qualifying experience working as a Speech and Language Therapist
