Contract Duration: Ongoing - to start ASAP, Start Date: 28/04/2025, initially for 3 months but likely to be extended
Working Hours: 35 hours per week, Monday to Friday, with hybrid working arrangements (3 days in the office)

We have an exhilarating opportunity for a motivated, qualified social worker to join the Lambeth Hospital Social Work Team, proudly based at St Thomas’ Hospital. Here, you'll be an essential part of a team that supports Lambeth residents admitted to Guy’s and St Thomas’ Hospital and other hospitals out of the borough. This role is all about delivering strength-based preventative support, assessing, establishing support structures, and safeguarding patients. Whether you're reviewing care packages or considering placements, you'll help facilitate safe and timely hospital discharges. What you will do:

- Deliver strength-based preventative support and conduct assessments ensuring patients have the necessary safeguards in place.
- Work closely with the multi-disciplinary team and voluntary sector organisations to achieve comprehensive care solutions.
- Manage referrals to reablement services to support patients in enhancing their daily living skills.
- Facilitate safe and timely discharge decisions, collaborating with various teams and agencies across the borough.
- Conduct assessments and reviews for adults over 18 with a range of physical health disabilities and vulnerabilities, ensuring adherence to safeguarding procedures.
