Main Duties:

  • Triage Safeguarding Concerns: Identify whether the Section 42 duty is met and ensure that risk management is evident, with appropriate actions taken.
  • Convene Multi-Disciplinary Meetings: Lead planning and risk assessment meetings for individuals with complex needs and high risk, ensuring a coordinated approach to safeguarding and supporting individuals to achieve their outcomes, with a clear focus on Making Safeguarding Personal (MSP).
  • Collaborative Working: Work jointly with other teams and statutory organisations, including the Police, Care Quality Commission, NHS bodies, care providers, and other Lincolnshire County Council (LCC) teams, to make enquiries and resolve safeguarding issues.
  • Risk Monitoring and Management: Take responsibility for monitoring and assessing risk once allocated a case, managing risks and making decisions within the legal framework and in accordance with the Lincolnshire multi-agency safeguarding policy and procedures for vulnerable adults.
  • Reporting and Presenting Evidence: Gather facts, prepare detailed reports, and present evidence at planning meetings, answering questions and ensuring all relevant information is provided.
  • Maintain Effective Communication: Ensure strong communication links within the County Council and with partner agencies.
  • Information Management: Manage confidential and sensitive information in line with General Data Protection Regulations (GDPR) and Information Governance procedures, promoting individuals’ wellbeing through appropriate information sharing.
  • Accurate Record Keeping: Ensure all activities are recorded accurately and maintained according to Lincolnshire Adult Care procedures and GDPR guidelines.
  • Office Attendance: Be office-based in Sleaford two days per week.
  • Safeguarding Visits: Conduct safeguarding visits across Lincolnshire.

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
