Are you ready to take on an exciting new role for the next 3 months? We are seeking an experienced Locum Social Worker to join the Adults Hospital Discharge Team in Macclesfield. This opportunity offers a full-time schedule from 9 to 5, allowing you to dive into fulfilling work while enjoying a balanced lifestyle. You'll be working primarily with the East teams, supporting discharges from out of area hospitals such as Wythenshawe, MRI, and Trafford, all while being based in a dynamic community team environment. This role demands an experienced individual who can operate at a fast pace, complete assessments, and handle capacity and best interest meetings with finesse.

What you will do:
- Support hospital discharges for adults within the East teams from hospitals such as Wythenshawe, MRI, and Trafford.
- Conduct thorough assessments and participate in capacity and best interest meetings to ensure appropriate planning for patient care.
- Collaborate with medical professionals and community resources to coordinate comprehensive discharge plans.
- Work efficiently in a fast-paced environment to meet the needs of patients and families.
- Uphold safeguarding practices and adhere to relevant policies and legislation.
