Main Duties:

  • Assessments and Casework: Conduct assessments, casework, support planning, and discharge planning as directed.
  • Risk Management: Follow the Positive Risk policy framework to identify and manage risks throughout the resident’s journey, working collaboratively with the resident and key individuals to support their chosen lifestyle wherever possible.
  • Personalised Support Plans: Develop outcome-focused, personalised support plans that promote the independence of vulnerable residents, identify their strengths, and help them enhance their family and social networks.
  • Resource Coordination: Mobilise and coordinate resources within the department and across other services, agencies, and the community. Liaise effectively with hospitals, community groups, statutory organisations (e.g., finance, placements, legal, NHS professionals), voluntary organisations, providers, and housing services to support assessment, casework, and planning.
  • Negotiation: Work with residents, their families, and carers to negotiate and meet their needs within available resources.
  • Reporting and Follow-Up: Prepare and present clear, concise reports for multi-agency and disciplinary meetings, ensuring all follow-up actions are completed within agreed timescales.
  • Safeguarding: Be vigilant in identifying instances where vulnerable residents may be at risk, and ensure safeguarding alerts are made within required timescales. Conduct safeguarding investigations, including risk assessments and protection planning.
  • Creative Commissioning: When family and social resources are insufficient, creatively commission services that deliver individualised, person-centred support while considering cost and value.
  • Therapeutic Approaches: Use a range of therapeutic methods to enhance coping skills, independence, and choice for vulnerable residents, their families, and carers.
  • Life History and Conflict Resolution: Consider the life history, losses, goals, and aspirations of residents, and work constructively to help families develop the strength and skills to resolve conflicts.
  • Support Plan Monitoring: Regularly monitor and review support plans with all stakeholders, ensuring the resident remains central to the process. Confirm that plans meet assessed needs, adhere to service standards, and are completed within statutory timescales.
  • Legal Compliance: Ensure all work is conducted in accordance with community care law and best practice guidance.
  • Mental Capacity Act Adherence: Adhere to the Mental Capacity Act by conducting assessments when necessary, referring to an Independent Mental Capacity Advocate (IMCA) as per the Act, or undertaking Best Interests decisions under the Deprivation of Liberty Safeguards (soon to be Liberty Protection Safeguards).
  • Case Conferences and Reporting: Prepare and present reports for case conferences, panels, tribunals, review meetings, and court appearances as required.
  • Supervision and Professional Development: Participate in regular supervision, staff appraisals, and meetings to ensure the delivery of effective services and ongoing professional development, including pursuing Post-Qualifying Awards.
  • Training and Best Practice: Engage in staff development training and performance reviews, keeping up to date with research and best practices relevant to the role.
  • Specialist Expertise: Develop specialist expertise in social work methods for supporting vulnerable residents, especially those from discriminated-against groups.
  • Professional Standards: Adhere to the Social Work England Standards of conduct, performance, and ethics.
  • Data and Strategy Cooperation: Assist in providing statistical and other information to assess the effectiveness of social services and contribute to achieving departmental strategic objectives.

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
