Duties:

  • Be responsible for conducting comprehensive Care Act assessments for individuals who have been discharged from hospital into a temporary 24-hour care placement
  • Assess the individual's care needs, identifying appropriate services, and coordinating the care and support
  • Play a crucial role in supporting individuals to achieve their outcomes
  • Conduct Care Act assessments of individuals who have been discharged from the hospital and require community support
  • Assess and evaluate the individual's physical, mental, and emotional needs, as well as their functional abilities, to determine the appropriate level of care and support required
  • Collaborate with individuals, their families, and other healthcare professionals to gather relevant information and develop comprehensive care plans
  • Provide advice, guidance, and emotional support to individuals and their families, helping them to understand their options and make informed decisions
  • Liaise with relevant community resources, such as healthcare providers and voluntary organisations, to ensure the seamless provision of services
  • Advocate for individuals' rights and entitlements, ensuring that they receive appropriate and timely support services
  • Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with organisational and legal requirements
  • Stay current with changes in legislation, policies, and best practices related to social work, care assessments, and community support services

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Must have D2A experience to be considered for the role
  • Valid driver's license and own car with business insurance
  • Proven experience in conducting assessments and developing care plans
  • Experience in Discharge to Assess is essential
  • Sound knowledge and application of the Care Act, Mental Capacity Act and Best Interest Processes is essential for this role
  • Knowledge and experience of Continuing Health Care processes and participating in DST's would be an advantage, but not essential
  • Excellent communication skills, both verbal and written, with the ability to convey complex information in a clear and empathetic manner
  • Effective organisational and time management skills, with the ability to prioritise tasks
  • Strong interpersonal skills and the ability to build rapport with individuals, families, and multi-disciplinary teams
  • Proficient computer skills, including experience with electronic record-keeping systems
  • 3 years reference, gaps in employment history checked
