What you will do:
- Work within a supportive and collaborative Connecting Care Hub, engaging with adults over 18 across the West side of the district.
- Use your knowledge of the Care Act 2014 to perform strengths-based conversations, determine eligibility, and support planning with service users, their families, and advocates.
- Conduct Capacity and Best Interest Assessments in line with the principles of the Mental Capacity Act 2005.
- Play an active role in NHS Continuing Healthcare, including completing checklists and participating in DST meetings.
- Effectively manage and prioritise a caseload in a fast-paced environment, ensuring high standards of service.
