Main Duties:
- Manage and Prioritise Workload: Effectively organise and prioritise tasks to meet deadlines, ensuring high standards of written documentation, interventions, and measurable outcomes.
- Person-Centred Assessments & Reviews: Conduct assessments and reviews in a holistic, person-centred manner that focuses on individuals' strengths, promoting preventative options to maximise independence and overall well-being.
- Carer and Family Support: Provide assessment, review, and support to carers and family members, ensuring their needs are addressed alongside the individual’s.
- Adherence to Mental Capacity Act: Ensure all assessments and reviews align with the Mental Capacity Act, making sound judgments regarding individuals who may struggle to engage. Facilitate advocacy support and perform Mental Capacity Assessments and Best Interest decisions where needed.
- Outcome-Focused Service Delivery: Ensure the assessment and review process is outcome-driven, holding service providers accountable for achieving agreed-upon goals for service users.
- Maintain Accurate Records: Keep full, clear, and accurate records of all interactions, assessments, and actions taken, ensuring compliance with legal and organisational standards.
- Manage Changes in Care Arrangements: Action changes to support plans promptly, present cases to panels for additional funding if required, work with brokerage to adjust care packages, and ensure care arrangements offer value for money.
- Work within the Care Act 2014 Framework: Ensure that all assessments and reviews comply with the principles of the Care Act 2014, focusing on accessibility, person-centred planning, and preventative care that enhances well-being.
- Active Participation in Supervision: Engage fully in supervision, preparing thoroughly and using these sessions for reflection and development.
- Collaboration on Continuing Health Care: Work within the Continuing Health Care framework, including completing Decision Support Tools (DSTs) and collaborating with health colleagues to ensure joint working and holistic care planning.
- Ensure Provider Quality Standards: Work closely with commissioning teams to monitor and ensure service providers meet required quality standards, raising concerns or issues when necessary.
- Promote Service User Safety: Collaborate with partners, providers, and carers to manage risks effectively and support positive risk-taking for the benefit of service users.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Adults Services within the UK
