Team/Specialism: Hospital Discharge
Pay Rate: £40 per hour (assignment rate)
Contract: Locum, 6-Months
Hours: Full-time

Requirements of the Social Work Lead:
- Degree or equivalent in social work
- Current Social Work England registration
- Social Workers with line management experience prep ASYE
- Hospital discharge experience
- Only applicants with consistent placement duration of 6 months + will be considered
- Needs to be prepared to be in county and on site at least 3 days a week
