What you will do:
- Assess and manage a diverse caseload, developing tailored care plans to meet the specific needs of individuals and families within the community.
- Collaborate with a multi-disciplinary team, including health professionals and external agencies, to provide holistic and comprehensive support.
- Conduct home visits and liaise with service users to ensure effective delivery of social care services.
- Facilitate workshops and meetings to empower service users and promote their wellbeing and independence.
- Monitor and evaluate the effectiveness of care plans, making necessary adjustments to ensure the best possible outcomes.
