Sanctuary Social Care is currently looking for an experienced and dedicated Practice Manager to fill a full-time role within an Adults Service based in Newham, within the Hospital Discharge Team.
The salary for this permanent Practice Manager job is £42,681 - £48,585 per annum.
Duties:
To undertake a key role in providing the professional lead in the delivery of social work
To ensure that the customers are at the heart of all our intervention; maximising choice, control and promoting independence
provide management support to our Social Workers to undertake assessments and formulate discharge plans for vulnerable residents and families
Arranging timely discharges, working in a joined up integrated way, to minimise delayed transfers of care from Hospital
Requirements:
A recognised and relevant professional social work qualification (Degree / Diploma in Social Work)
Qualified HCPC registered Social Worker
Management experience is essential
Must have substantial experience of developing staff and raising standards of professional practise in busy operational Practices providing direct work to vulnerable adults and their carers
Must have up-to-date knowledge of relevant best practice, legislation, guidance and regulations and you'll be skilled at building relationships with stakeholders and colleagues
