What you will do:
- Conduct comprehensive assessments using standardized tools and holistic approaches to formulate integrated care plans.
- Perform detailed falls risk assessments and collaborate with multidisciplinary teams to create effective treatment strategies.
- Work within the Transfer of Care Hub to support the Discharge to Assess process at home from the hospital and community beds.
- Maintain clear and effective communication with all members of the healthcare team to achieve the highest patient care goals.
- Provide supportive and informative guidance to patients and their families, promoting their health and recovery.
- Requirements of the Physiotherapist:
- Possess a qualified Physiotherapist status with a degree or equivalent within Physiotherapy.
- Recent UK experience within specialised physiotherapy, with NHS experience advantageous but not essential.
- Registered with the HCPC as a physiotherapist, ready to work in the UK.
- A valid driving licence required for community working.
