What you will do:
- Work collaboratively as part of a specialised physiotherapy team within the Community Integrated Neuro and Stroke Service alongside professionals including Occupational Therapists Speech and Language Therapists Dietitians Psychologists Assistant Practitioners and Rehabilitation Technicians.
- Deliver comprehensive evidence-based assessments and treatments for adults with neurological disorders in their own homes at clinics or within other community environments in Coventry.
- Adhere to established stroke and neurological care pathways ensuring all rehabilitation is conducted according to specified protocols.
- Coordinate closely with team members and other community services to foster effective rehabilitation for patients.
- Contribute to quality and activity monitoring systems enhancing service delivery.
- Follow the lone worker policy ensuring safety for all during community visits.
- Provide flexible service coverage over weekends and bank holidays meeting the needs of patients.
- This position requires HCPC registration and a valid driving licence to travel across service locations in Coventry.
