What you will do:
- Work across ICU, respiratory, cardiology, enhanced care, and surgical wards, applying your expertise to manage complex cases.
- Plan and facilitate efficient discharge processes for patients transitioning from ICU step-down care.
- Autonomously handle the rehabilitation of post-ICU patients, ensuring comprehensive care plans are implemented.
- Collaborate flexibly with your multidisciplinary colleagues to provide exceptional patient outcomes.
Requirements:
- HCPC registration.
- Experience in acute healthcare inpatient settings, with a strong focus on respiratory, surgical, cardiology, neuro, and neuro-rehab.
- Proven discharge planning capabilities.
- Desirable experience within an ICU setting.
