Duties:
- Screening Oncology and Haematology prescriptions for the Chemotherapy day unit and Haematology day unit
- Assist in providing a ward based and day care specialist pharmacy cancer service to haematology and oncology cancer patients
- Assist in developing, providing, managing and monitoring a safe and effective clinical pharmacy service to haematology and oncology cancer patients
- Provide highly specialist pharmaceutical input to these patients, working with the multidisciplinary team
- Participate in risk management by actively mitigating medication errors in the service
- Support providing supervision, support and clinical training for pharmacy colleagues and other healthcare professionals around medicines management for cancer services when needed
- Make cost-effective recommendations without compromising patient care
- Deputise for the highly specialist pharmacists in their absence
- Communicate complex drug or medicine related information to patients & relatives in prescription medication counselling, drug history taking and reviewing prescription
- Communicates with medical and nursing staff regarding medicine management issues whereby persuasive skills are required for practicing against evidence based medicine/formulary guidelines
- Participates and discusses within the multidisciplinary team on a specific ward the provision of drug related information and assessment of patient’s treatment
Requirements of the Pharmacist:
- GPHC Registration
- NVQ or equivalent – Pharmacist qualification
- Be working towards or completed the SACT passport and have experience in Oncology and/or Haematology
