Main duties of the Community/District Nurse:

  • You will be working as part of the Primary Care at Home team predominantly on Unplanned Care to triage and assess referrals into the service
  • Be accountable for initial history taking, clinical assessment, triage and onward referral
  • Expected to be flexible to cross cover 
  • Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions
  • Working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community

Requirements of the Community/District Nurse:

  • RGN qualified/NMC registered with valid live pin
  • Recent UK nursing experience
  • Must have Community/District nursing experience
