Banding/Grade: Band 6

Pay Rate: £26-£40per hour (assignment rate)

Role purpose:

  • Provide high standards of care to patients with highly complex continuing care needs. Care will be delivered within the patient’s own home / other community setting or local hospital.
  • Required to deliver care independently but will be expected to report back to other members of the team, the patient and family.
  • The post will involve lone working; there is always a qualified nurse on-call for support.

Main duties:

  • Maintain confidentiality on all matters relating to patients/families adhering to Trust policies.
  • Assess patient care needs. To implement and evaluate programmes of care with patients and their families.
  • Ensure that the delivery and quality of care given complies with local and Trust agreed policies and procedures.
  • Communicates effectively with patients/families/other members of the multi-disciplinary team to ensure prescribed plan of care is delivered.
  • Ensure that patients receive high standards of personal care.
  • Keep accurate and legible patient records adhering to Trust policy.
  • Works in patient’s homes without direct support in complex circumstances, maintaining professional boundaries and integrity at all times adhering to the local Code of Conduct.
  • Working with patients who have life-limiting illness and therefore frequent exposure to difficult and sometimes distressing experiences due to intensity of this role.
  • Liaises with medical / paramedical staff and other team members to ensure timely delivery of prescribed care. Communicate any changes to the patient, family and On Call nurse as required.
  • Provide emergency care for each patient. To be aware of how and when to instigate emergency contingency plans and procedures.
  • Actively involved in physiotherapy for each patient, if appropriate, as planned in their individual care plan.

Requirements of the Complex Care Community Nurse:

  • RGN qualified/NMC registered with valid live pin
  • Recent UK nursing experience
  • Must have Community/District nursing experience
