What you will do:

  • Provide daily duty cover to support the team and manage caseloads efficiently
  • Offer ongoing management and support to our functional patients
  • Address the behavioural and psychological symptoms in dementia patients within the Older Adults CMHT
  • Collaborate with a multidisciplinary team to deliver comprehensive care plans
  • Maintain accurate patient records and ensure compliance with health care regulations
  • Conduct regular home visits and assessments to monitor patient progress
