What you will do:
- Provide daily duty cover to support the team and manage caseloads efficiently
- Offer ongoing management and support to our functional patients
- Address the behavioural and psychological symptoms in dementia patients within the Older Adults CMHT
- Collaborate with a multidisciplinary team to deliver comprehensive care plans
- Maintain accurate patient records and ensure compliance with health care regulations
- Conduct regular home visits and assessments to monitor patient progress
