What you will do:
- Conduct comprehensive assessments, triages, and interventions aligning with service users' needs.
- Perform risk assessments and facilitate access to a range of mental and physical health interventions.
- Play a leading role in the Multi-Disciplinary Team by facilitating care planning and supervising junior staff.
- Undertake a lead role in holistic assessment and care for patients with complex health and social care needs.
- Provide expert advice and support to both service users and providers, aiding in the integration of support from primary and secondary care services. Implement and monitor care plans ensuring they are responsive to patients' needs and adjusting as necessary.
