What you will do:

- Conduct comprehensive medicine reconciliation for patients in the Emergency Department (ED) and Admissions Unit. Ensure each medication history is meticulously accurate.
- Identify and resolve any medication discrepancies by effectively liaising with patients, carers, General Practitioners, and community pharmacies.
- Lead efforts in medicines optimisation, guaranteeing correct prescribing, administration, and storage procedures.
- Facilitate the swift supply of medicines, significantly reducing delays in patient care.
- Support patient discharge planning, maintaining accurate medication records and ensuring safe prescribing practices.
- Provide thorough counseling and advice to patients and their carers about newly prescribed medications.
- Collaborate with pharmacists, nurses, and clinicians to boost medication safety and patient adherence.
- Supervise and mentor junior pharmacy technicians and assistants.
- Play a key role in training and educational programs for pharmacy staff, nurses, and medical teams.
- Drive quality improvement initiatives, focusing on reducing missed doses and medication errors.
- Support the rollout and use of electronic prescribing (EPMA) systems.
- Maintain strict adherence to medicines management policies and governance frameworks.
