What you will do:
- Work with residents to set up urgent care bridging, collaborating effectively within a multi-disciplinary team.
- Manage a caseload of 5-7 cases, ensuring timely progression within a maximum of 10 days.
- Perform in-depth Care Act Assessments and create comprehensive Care and Support Plans.
- Engage in safeguarding activities, handling concerns swiftly and efficiently.
- Apply your skills in completing Mental Capacity and Best Interests decision making.
- Facilitate the transfer of cases to Reablement, ensuring continuity of care.
- Liaise with health colleagues including Occupational Therapists, Physiotherapists, and Nurses to stabilise residents.
We seek a qualified and registered Social Worker with at least 2 years’ experience. Familiarity with fast-paced services, such as urgent response or hospital discharge, is desirable. Additionally, an understanding of the Reablement ethos and strengths-based practice will stand you in good stead.
